Vaccination Summary

Your pet’s vaccination certificate can be emailed to you or anywhere else you request within 24 hours of your request. This summary is usually requested by boarding and grooming facilities and can include the results of your pet's latest fecal test if needed.

Records Request

To obtain your pet's complete medical history, please call our office at (206) 932-3308. A complete record is usually needed when transferring your veterinary care to another hospital, being seen by a specialist, or when moving. Once you call in your request, we have a doctor look over your pet's record to make sure that everything is complete, and then it can be faxed or emailed to you or anywhere else you request.
